San Dimas Community Hospital Opens “Center for Senior Care”
(San Dimas, CA – February 25, 2020) San Dimas Community Hospital recently announced the grand opening of their “Center for Senior Care” inpatient program, led by Board Certified Geriatrician, Jhujhar, Singh, DO, along with a dedicated team of experts with specialized training and experience in geriatric healthcare.
Patients 65 years and older have unique medical needs, and the doctors and staff at San Dimas Community Hospital not only recognize those needs but are focused on proactively protecting and preserving quality life for the region’s older patients. The inpatient program is designed to provide specialized care tailored to meet the needs of the community’s most vulnerable senior patients. “Upon arrival, senior patients are evaluated using an evidence-based tool called Identifying Seniors at Risk (ISAR) to determine if the patient meets the criteria” says Dr. Singh. The unit is differentiated by having four newly renovated rooms with accommodations made for seniors staffed by geriatric trained nurses and physicians. Every detail of the Center for Senior Care Suites has been considered from the patient’s perspective to support the delivery of care in a setting that is safe, private and calming for seniors. “These suites have soothing two-toned paint, which is both calming and serves to reduce the risk for falls. They also feature a top-of-the-line TruRize chair that promotes safe patient mobility, larger clocks, phones, and larger televisions for entertainment purposes,” says Parrish Scarboro, Chief Executive Officer.
Patients in the Center for Senior Care will not only be treated by a Board-Certified Geriatrician, they will also have a medication consultation with a pharmacist, dietician oversight and education, an individualized assessment and care plan, and coordinated care for discharge to home or a senior living community.
