The American Heart Association presents the Get With The Guidelines® – Heart Failure and Stroke Gold Plus awards for proven dedication to ensuring all patients have access to best practices and life-saving care

SAN DIMAS, CA, Aug. 19, 2025 — San Dimas Community Hospital, a proud member of Prime Healthcare, has received the American Heart Association’s Get With The Guidelines®– Stroke Gold Plus and Heart Failure Gold Plus achievement awards, recognizing its commitment to following the latest research-based guidelines for the treatment of heart disease and stroke. Consistently applying these evidence-based protocols leads to more lives saved, shorter recovery times, and fewer hospital readmissions.

Heart disease and stroke are the No. 1 and No. 5 causes of death in the United States, respectively, according to the American Heart Association 2025 Statistical Update. Studies show patients can recover better when providers consistently follow treatment guidelines.
